Dictating Clinical Notes by Voice When Audio Must Stay on Your Phone

Yes, you can dictate notes after a consultation without the audio leaving your phone. YOURIO uses your phone's on-device speech recognition and never saves or sends the recording, and Magic · Standard cleans up the text offline. For patient details, decide on Memory first (turn it off or use Private mode), and use cloud AI cleanup only if your organisation's rules allow it.

The ten minutes after the patient leaves

The door closes, the next patient is due in ten minutes, and the details of this visit are still clear in your head: what they reported, what you examined, what you agreed to do next. Saying it would take one minute. Typing it on a phone takes five, and writing it up at the end of the day means writing it from memory.

Most clinicians, counsellors and therapists have thought about dictating. What makes them hesitate is simple: they're not sure where the audio goes, and patient details are exactly what they shouldn't hand to a service they haven't checked.

Why the usual options feel uncomfortable

  • Cloud dictation sends audio to a server for recognition. Some services handle it carefully — processing in real time and discarding it afterwards — but the patient details still travel to a third party, and your organisation may not have approved that service.
  • A voice memo to type up later creates an audio file with patient details in it, which then sits on your phone, in a backup, or in a shared folder until someone deletes it.
  • Typing it all later keeps everything local, but you lose the detail you had right after the visit.

How YOURIO handles it

YOURIO is a voice keyboard, so you dictate straight into whatever app you write notes in. Speech becomes text on the phone, using the on-device recognizer built into its system. The audio is processed as you speak and never recorded, saved or sent to us. Once your language is downloaded in the Speak tab, this works without a connection.

With Magic · Standard on, YOURIO also tidies the text on the phone: it removes filler words, keeps only your final self-correction and adds punctuation. For example:

  • You say: "um patient reports headache for three days, no wait, four days, uh, worse in the mornings, no fever"
  • YOURIO types: "Patient reports headache for four days, worse in the mornings, no fever."

Nothing in that step touches the cloud. For more on how filler-word cleanup works, see voice typing without the "um".

Decide what stays on the phone

Memory is YOURIO's private timeline of what you've dictated. It keeps text only, never audio, and only on the phone. It is also on by default, which means patient details you dictate are kept there unless you choose otherwise. Three options:

  • Turn Memory off. YOURIO simply types. Even then, the personal dictionary can learn from your corrections, but it keeps only the word pair (a misheard term and your fix), not sentences.
  • Use Private mode while you write notes. It pauses both saving and learning, and there is a button for it on the keyboard, so you can switch it on for clinical notes and off for everything else.
  • Keep Memory but clean up. Delete single records from the Timeline, or everything in Settings. Uninstalling the app removes all of it, because there is no other copy.

If you want AI to restructure the note

Magic · Standard won't reorder your thoughts. Magic · Enhanced will — it reorders, fixes misheard words and turns lists into bullets — but it sends text (never audio) to a cloud AI. Before using it with patient details, check your organisation's rules. If they allow a specific provider, add your own API key and the text goes straight from your phone to that provider. If no outside AI is allowed, run a model on a computer you control; see self-hosted AI dictation. The full data flow is in Does voice typing upload your voice?

Honest limits

  • No compliance claims. YOURIO doesn't claim any certification for health information. It tells you what the app does; your organisation decides whether that fits its rules.
  • Medical terms depend on your phone's recognizer. On-device recognizers can miss drug names and jargon. Add the terms you use to the personal dictionary by hand, and read every note before you save it.
  • It types notes; it isn't a records system. YOURIO doesn't record consultations, transcribe the patient, or connect to clinical software. It types what you dictate wherever your cursor is.
  • Your phone is the boundary. If Memory is on, anyone who can open your unlocked phone can read it. Use a strong screen lock.

Set it up before your next clinic

  1. Install YOURIO (download links are at the end of this article) and turn on the YOURIO keyboard.
  2. Download your language in the Speak tab, then test it once in airplane mode.
  3. Choose Memory off, or plan to use Private mode for notes.
  4. Add the drug names and terms you use to the personal dictionary.
  5. After the next visit, dictate the note into your notes app, then read it through before saving.

Frequently asked questions

Can I dictate patient notes without sending audio to the cloud?

Yes. YOURIO recognizes speech with your phone's on-device recognizer, so the audio stays on the phone and is never saved or sent to YOURIO. Magic · Standard removes fillers and adds punctuation on the phone as well. Only optional cloud features such as Magic · Enhanced send text, and only when you set them up.

Does YOURIO keep patient details on my phone?

Only if Memory is on, and Memory is on by default. It keeps the text you dictate, never audio, only on your phone. You can turn Memory off, use Private mode while you write notes, or delete records from the Timeline or Settings.

Is YOURIO approved for medical records?

YOURIO makes no certification or compliance claims. It documents what it does: audio stays on the phone and isn't stored, Memory stays on the phone and can be turned off, and cloud AI is optional and text-only. Check your organisation's rules before using it for patient information.

Will it recognize medical terms and drug names?

That depends on your phone's on-device recognizer, which may miss specialist terms. You can add words to YOURIO's personal dictionary by hand, and when you correct a word YOURIO has just typed the same way twice, it is added automatically.
